WHAT WE DO

Referral management software built for care coordination.

ICC is the EHR-agnostic platform that carries a referral from creation to closure — connecting the sending and receiving provider on one shared workflow.

Sending a referral is easy. Knowing what happened to it isn’t.

Most practices can already send a referral — by fax, by portal, or through a module in their EHR. What almost none of them can do is confirm the patient was scheduled, know whether the visit happened, or get the consult note back into the referring provider’s workflow without someone picking up a phone.

That gap isn’t a communication problem. It’s an infrastructure problem: the sending and receiving practices are working in different systems, and nothing sits across them. ICC is that layer.

WHAT ICC DOES

Five things that have to work for a referral to close.

1

Build the network you actually want to work with

Networks configure their own referral partners by initiative and by insurance plan — specialists, post-acute, behavioral health, and anyone else who belongs on the care team. Practices see the providers relevant to that patient’s coverage and needs, not an undifferentiated directory.

2

Interoperate with your EHR — or integrate with it

ICC automates referral creation from any certified EHR, pre-populating clinical and administrative fields from a CCDA and attaching supporting documentation such as labs and images. Infina Connect’s integration technology enables bidirectional connectivity with certified and non-certified EHRs without requiring the EHR vendor’s involvement.

3

Match patients to the right services

The care optimization module combines a Social Determinants of Health screening with a risk stratification tool. Based on clinician input, ICC identifies the services a patient may be eligible for, matches those services against the profiles of providers in the network, and factors in the distance between provider and patient — so transportation and logistics don’t quietly derail the referral.

4

Coordinate every open referral from one place

A centralized dashboard gives an instant view of referral status across all patients, so staff can work by acuity rather than by inbox order. Referral management, tracking, and follow-up happen in one workflow instead of across fax logs, spreadsheets, and phone calls.

5

Keep both providers connected until the loop closes

Mutual status visibility, real-time secure messaging, and notifications between referring and consulting practices — so the consult note comes back, the referring provider knows the outcome, and the record of care is complete on both sides.

Built around your high-value network

A referral rarely moves in a straight line. A patient may see a specialist, get imaging, land in a health system, and pick up post-acute or community services along the way.

You can’t succeed in value-based contracts if you don’t know where your patients are — and you can’t know where they are without tracking referrals across the network. Without coordination, you’re subcontracting out the care of your patients with no view into the quality they receive or what it costs. ICC gives you that view.

A proven implementation and a team that stays

ICC is delivered as software-as-a-service, with access and training designed to be straightforward for practice staff who didn’t ask for another system to learn.

Whether you’re a single practice or an enterprise network spanning dozens of community partners, communicating the change is as important as the software itself. Our team works alongside yours through the whole rollout.
