Three places where coordinated referrals change the economics
The organizations that use ICC have one thing in common: they carry responsibility for what care costs and how it turns out, not just for the visit in front of them. Where they differ is which network they’re coordinating, and what they’re trying to prevent.
Value-based care networks
ACOs, IPAs, and health systems coordinating referrals to control downstream cost and quality.
Behavioral health
Health systems reducing avoidable ED boarding, readmissions, and bed days.
Direct provider contracting
Payers, employers, and self-insured plans using bundled, flat-rate provider networks.
VALUE-BASED CARE NETWORKS
ACOs, IPAs, and health systems
You can’t succeed in value-based contracts if you don’t know where your patients are — and you can’t know where they are without tracking referrals across the network. Without coordination, you’re subcontracting out the care of your patients with no view into the quality they receive or what it costs.
ICC gives you that view. Practices coordinate referrals inside the network you’ve built, clinical records travel with the patient, and every referral is tracked from creation to closure — so the organization accountable for the total cost of care can actually see it.

“Infina Connect’s Intelligent Care Coordinator is the most important tool that we use. It has been instrumental in our ACO’s ability to achieve over $80 million in total savings in the last 3 years.”
BEHAVIORAL HEALTH COORDINATION
Health systems and emergency departments
Behavioral health patients wait longest for care and are placed least reliably. Emergency departments hold them for days without medical necessity, reimbursement is forfeited, and too many patients never reach the treatment they need at all.
Breaking that cycle takes a different way of partnering with the behavioral health community — a defined network, clear criteria for what each provider can take, and visibility into whether the placement actually happened.

DIRECT PROVIDER CONTRACTING
Payers, employers, and self-insured plans
When a plan contracts directly with providers — bundled procedures, flat-rate pricing, a defined preferred network — the arrangement only works if patients actually reach those providers and the plan can see that they did.
ICC makes the contracted network visible inside the referral workflow. Practices see which providers are in the arrangement for that patient’s coverage, the referral is coordinated to them, and the plan gets confirmation the care was delivered. Infina Connect supports arrangements ranging from regional direct-contracting networks to a statewide plan covering public employees.
